Maryland Department of Aging: Maryland Access Point

Maryland Access Point (MAP) is a statewide  resource for information and assistance about  long-term services and supports, particularly for  older adults and individuals with disabilities, regardless of income.  Information and resources are available to address a variety of needs, health, transportation, income and financial aid, senior citizen centers and clubs, daily meals, pharmacy assistance, housing, volunteer opportunities, and more.

There are 20 local MAP Information and Assistance offices throughout the state.  Trained staff can provide assistance in determining needs for services and make referrals to appropriate agencies, including case management and coordination for persons requiring ongoing services.

VA Healthcare: Community Living and Geriatric Care

The Loch Raven VA Community Living and Rehabilitation Center specializes in providing rehabilitation and care for patients in the VA Maryland Health Care System.  The Center provides a number of services including physical and occupational therapy.  Hospice and nursing home care is provided, as well as treatment for patients with dementia and Alzheimer’s disease.

The Center is located on Loch Raven Boulevard in Baltimore, MD.

The Martinsburg VA Medical Center offers Geriatric and Long-Term Care for patients seen by their Medical Center.  Geriatric and Long-Term Care provides services for veterans who are elderly and have complex needs, and veterans of any age who need daily support and assistance. Veterans can receive care at home, at the medical center or in the community.

The Washington DC VA Medical Center offers Geriatric and Long-Term Care as well for patients seen by their Medical Center.  Services include palliative care, geriatric medicine, community living center, and primary care.

If you are local to Anne Arundel or Howard County, this organization provides support to people with disabilities allowing them to live independently at home.

Accessible Resources for Independence, Inc.

Accessible Resources for Independence, Inc., is a center for independent living that provides support and services to people with disabilities in Anne Arundel and Howard counties in Maryland. It is a consumer-controlled, community-based, cross-disability, and nonresidential private, nonprofit agency.

For veterans, Options Counseling through the Veterans Directed Home and Community Based Services are offered.

The program provides Veterans the opportunity to self-direct their long-term supports and services and continue to live independently at home. Eligible Veterans manage their own flexible budgets, decide for themselves what mix of goods and services best meet their needs, and to hire and supervise their own workers. Through an Options Counselor, the Aging & Disability Network provides facilitated assessment and care/service planning, arranges fiscal management services, and provides ongoing options counseling and support to Veterans, their families and caregivers.
